Long Term Care

Martin Luther Long Term Care Center is located in a residential setting on beautifully wooded grounds. Our “neighborhoods” – Fox Crossing and Eagle Crest – encourage a sense of community, helping to foster friendships with residents and staff. An interdisciplinary care team – social work, therapy, dietary, therapeutic recreation, and nursing – works with each resident, their family and heath care providers to develop a personal plan of action.

  • Individualized care and service plans developed with team approach – therapists, doctors, nurses, social workers, dieticians, therapeutic recreation and spiritual care
  • Partnership with in-house physician/nurse practitioner care teams
  • Team of social workers to process admission paperwork, answer questions

  • Physical, occupational, speech and aquatic therapy available on-site

  • Podiatry, dentistry, audiology, and optometry care on-site by appointment

Transitional Care/Therapy

Transitional Therapy Care plays an important part in regaining lost motor function after surgery, a serious fall, illness, stroke or other medical issues. The Transitional Care Unit (also known as Short Term Therapy or Rehab) provides patients with a customized program that may include extensive physical, occupational, speech or aquatic therapy, helping to pave the way for a speedy recovery. Therapy is also available as an outpatient to help continue improvement. Our experienced professionals strive to deliver quality, value, and positive outcomes to our parents.

  • 24-hour admissions, 7 days a week
  • “One stop shop” for discharge planners – admissions coordinator collects pertinent information, completes clinical review and verifies insurance coverage
  • Team of social workers to process admission paperwork, answer questions during patient recovery and assist with services a patient may need to make a successful transition back to their home
  • Therapy – physical, occupational, speech and aquatic – available 7 days a week
  • All therapy staff trained on ultrasound, e-stim and diathermy for a number of conditions including stroke recovery, pain, urinary incontinence, falls, and muscle tightening; plus certified lymphedema therapists
  • Interact/SBAR system approach to reduce re-hospitalization
  • Pharmacist on-site to review patient medications
  • Interdisciplinary team – therapists, doctors, nurses, social workers, dietitian, therapeutic recreation, and spiritual care – help promote recovery
  • Partnership with in-house physician/nurse practitioner care teams

Adult Day Clubs

Adult Day Clubs offer seniors social activity and care during the day while allowing them to return to their home at night. Daily socializing can lead to a happier and more satisfying life; these activities challenge the mind and body, which may help ward off and slow the progress of memory loss.

Exercise, spiritual services, snacks & meals, music, and special outings are just some of the activities Club members can enjoy. Important services like bathing, medication management, and therapy – physical, speech or occupational – are offered for a fee. Choose from either Creekside Club, located at the Creekside Community Center, or Campus Club located at Martin Luther Campus. Both offer full or half day programs, Monday through Friday, with door-to-door transportation available.
