Short-Term Rehabilitation

Easing the Transition from Hospital to Home

Augustana Care’s short-term rehabilitation offers the services of professionals at Centrex Rehab who can help speed recovery and focus on working to restore patients’ strength and abilities.

The treatment we provide is as varied as the people we see. Some come to us after surgeries and neurological traumas, while others are dealing with respiratory conditions or other illnesses.

Rehabilitation services typically offer both semi-private and private rooms with flat-screen televisions, phones and larger restrooms. Computers, a coffee shop and visitor’s lounge are also available. A range of treatment options may include occupational, physical and speech therapy; and Augustana Care works closely with attending physicians to help ease the transition from hospital to home.

Skilled Nursing

Augustana Care’s skilled nursing care is based on our belief that mind, body and spirit are intricately connected. Therapeutic recreation, spiritual care and life-enriching activities are an integral part of life here, along with twenty-four-hour care provided by licensed nurses.

The Augustana Care team works with residents and family members to create and coordinate effective care plans, answer questions and maintain frequent communication. Our goal is to provide the greatest comfort and quality of life possible for every resident.
