Skilled Nursing

Augustana Care’s skilled nursing care is based on our belief that mind, body and spirit are intricately connected. Therapeutic recreation, spiritual care and life-enriching activities are an integral part of life here, along with twenty-four-hour care provided by licensed nurses.

The Augustana Care team works with residents and family members to create and coordinate effective care plans, answer questions and maintain frequent communication. Our goal is to provide the greatest comfort and quality of life possible for every resident.

Short-Term Rehabilitation

Easing the Transition from Hospital to Home

Augustana Care’s short-term rehabilitation offers the services of professionals at Centrex Rehab who can help speed recovery and focus on working to restore patients’ strength and abilities.

The treatment we provide is as varied as the people we see. Some come to us after surgeries and neurological traumas, while others are dealing with respiratory conditions or other illnesses.

Rehabilitation services typically offer both semi-private and private rooms with flat-screen televisions, phones and larger restrooms. Computers, a coffee shop and visitor’s lounge are also available. A range of treatment options may include occupational, physical and speech therapy; and Augustana Care works closely with attending physicians to help ease the transition from hospital to home.

Hospice Care

Grace Hospice care encircles you with comfort, pain management, honesty, and respect for your decisions. Fostering peace and grace, we emotionally and spiritually support you and your loved ones.

Hospice is a philosophy of care that recognizes every person deserves to live out his or her life with respect and dignity, free of pain, and in an environment that promotes quality of life.Hospice begins with a referral from a medical provider when life expectancy is six months or less. This specialized end-of-life care focuses on comfort with services that include medical, personal care, emotional support, and spiritual care.

We serve the person wherever they consider home. This may be an assisted living community, skilled nursing, or private home.

Care Suites

For those requiring a higher level of physical care in assisted living settings, care suites offer reassurance and support. You’ll find the same amenities and services provided in traditional assisted living, enhanced by specialized features. Staff is trained to provide focused, individualized attention to residents.

Assistance with daily activities like dressing, bathing and getting to and from meals are all included for care suite residents. Whatever your needs are, you can take comfort in knowing that caring professionals are close at hand.
