Transitional Care Unit

Transitional care at Providence Place is for those who need to regain strength and independence after surgery, injury or illness and are in transition from home or hospital. Our experienced clinical team provides a spectrum of rehabilitation and therapy services on-site to restore individuals to their fullest potential.

Skilled Nursing

Skilled nursing at Providence Place is for residents who need round-the-clock assistance and medical attention on either a long-term or short-term basis. Skilled nursing is provided by professional nurses and nursing assistants. Highlights include restorative nursing, physical therapy, occupational therapy, speech and language pathology, social services, medications, supplies, equipment, and other services necessary for the health of our residents.

Memory Care

Memory care at Providence Place is for those with memory impairments such as Alzheimer’s and dementia as well as complex medical needs. Providence Place memory care includes the same level of support found within our skilled nursing care, with the added benefit of customized activities to maximize independence.
